Here is an Ault plugin class 2 triple output power supply. 120Vac 60hz 180mA input rating . Outputs are +12Vdc and -12Vdc at 130mA , +5Vdc at 500mA. Standard 3 pin ac plug is 2 7/8 in by 2 11/16 in.Cable is 7ft long and terminates in 5pin din connector. This is a great little supply for IC projects using linear and digital components. It is new, old stock.
