Serial 659208. Has a simple schematic as well. Made by Jackson Electronic & Mfg. Co in Ohio, USA (JAELCO).
Bottom has 2 tabs for mounting (measures 2 3/8" middle of hole to hole). 2 prong AC plug. 1K load resistor.
TESTED FORKS FINE. AC cord has some electrical tape on one section like a repair.
